MELD score (Model for End stage Liver Disease) is a score used to calculate the 90 day survival chances for people suffering with ESLD.The MELD score is used as part of the criteria for a Liver Transplant.
You will find the calculator here:
Meld Calculator
The score ranges from 6 which is the least sick to 40 which is pretty much terminal.
Most transplants are done when the average MELD score is around 20, but this can vary from state to state.
To get on the transplant list, most states require a MELD of 15.
MELD is NOT an indicator of how sick a person is, many people will die through complications with low MELD scores.
The Meld score requires results from blood tests which are:
Bilirubin which measures how effectively the liver excretes bile.
INR which measures blood clotting facors.
Creatinine which measures kidney functions.
Serum Sodium which measures the amount of sodium in the blood.
Note: The normal range of values for these are:
bilirubin 0.1 – 1.3 mg/dL
Creatinine 0.7 – 1.2 mg/dL
INR 0.8 – 1.2 seconds
Serum Sodium 135 to 145 mEq/L.
Of the three values for MELD, a doubling of Creatinine or INR produces a significant change in MELD.
Three blood tests – the bilirubin, the prothrombin time (PT) – measured as international normalized ratio (INR), and the creatinine (a measure of kidney function) are used to determine this value. The MELD score is calculated using the following equation:
MELD = 3.8 x log (e) (bilirubin mg/dL) + 11.2 x log (e) (INR) + 9.6 log (e) (creatinine mg/dL)
Formula :
MELD − Na − [0.025 × MELD × (140 − Na)] + 140
Explanation of Result :
3 Month Mortality :
MELD score > 40 : 71.3% mortality
MELD score 30–39 : 52.6% mortality
MELD score 20–29 : 19.6% mortality
MELD score 10–19 : 6.0% mortality
MELD score < 9 : 1.9% mortality.
